CONSENT TO PARTICIPATE Form - Six Flags Field Trip

Having reviewed the submitted petition from Class 90 for the relaxing/suspension of policies for our Field Trip and having reviewed Six Flags’ policies, we have crafted the following expectations for our trip .
In order to go on the Six Flags trip, Corpsmembers must agree to all the following (agreement will be indicated by indicating “Yes” or “No” and submitting your response. Should you chose not to respond we will assume you will not abide by the following and will not be allowed to attend):
Corpsmembers are highly encouraged to bring a water bottle (no glass) which may be refilled in the park.
Corpsmembers must leave all bags, fanny packs, backpacks, purses, etc. at the Youth Corps building. All personal items being taken must be kept in pockets or turned over to Thea or David to carry throughout the day.
All vapes must be turned over to Thea and David. No keeping vapes on your person.
Corpsmembers may wear personal shorts.
Corpsmembers must wear and keep their Youth Corps uniform shirt on the entire day.
We will split into a maximum of two groups. You must stay with at least one staff member (Thea or David) the entire time. We will wait for you at the exits of rides that we do not go on.
Corpsmembers will be able to keep their phones with them for the duration of the day.
Violating any of these conditions will result in termination from the program and loss of stipend. Being terminated from the program means you will not be invited to Completion Day or Graduation and you will not receive follow-up services from Youth Corps (such as extra academic help for those who have not yet received their diploma).
*Please note: if a Corpsmember chooses not to go on the trip, they will stay home for the day (Stipend will not be affected) and they will remain a Corpsmember in good standing. This means we expect to see them tomorrow for Completion Day, they will be invited to Graduation, and they will be eligible for follow-up services.
